Johnson & Johnson's third-quarter profit collapsed 95.9% to $210 million, with diluted earnings per share falling to $0.09 from $2.12 a year earlier, after the company booked a one-time special charge tied to its proposed resolution of ovarian talc litigation, according to figures the company reported in its quarterly announcements. Despite the near wipeout of reported net income, J&J raised its full-year 2026 sales and adjusted-earnings guidance, citing what it described as strong operational performance.

Talc Charge Dominates the Headline Numbers

The pre-tax litigation-related special item totaled $5,519 million in the quarter, compared with just $15 million in the same period last year, according to the company's reported figures. On a nine-month basis, the litigation-related adjustment reached $6,116 million. J&J said the $0.09 reported EPS figure reflects this one-time charge for the proposed talc litigation resolution along with costs tied to the acquisition of Firefly Bio, Inc. and a strategic collaboration with Sail Biomedicines.

Stripped of the charge and the deal-related costs, adjusted earnings told a different story: adjusted EPS fell a comparatively modest 7.5% to $2.59 per diluted share, while adjusted net earnings declined 6.8% to $6,339 million. Both the $25.4 billion in reported revenue and the $2.59 adjusted EPS figure beat Wall Street expectations of $25.28 billion in sales and $2.54 in adjusted earnings, according to CNBC, which cited Reuters and LSEG data. Our calculation shows the adjusted EPS beat amounted to $0.05, or roughly 1.97% above the $2.54 consensus estimate (2.59 − 2.54 = 0.05; 0.05 ÷ 2.54 × 100 ≈ 1.97%).

Guidance Raised Despite the Charge

Even with the charge weighing on reported results, J&J lifted its full-year 2026 outlook. The company now estimates reported sales of about $101.2 billion, or 7.4% growth at the midpoint, according to its own quarterly materials. CNBC reported that figure is up from a prior midpoint of roughly $101.1 billion. J&J also raised its adjusted EPS guidance by $0.06 to a new midpoint of $11.10, up from $11.04 previously, representing projected growth of 2.9% at the midpoint.

Chairman and CEO Joaquin Duato said the company's "strong third-quarter performance and increased sequential growth reflect the accelerating momentum of our focused portfolio," adding that this progress, together with strategic investments in high-growth areas, gives J&J "clearer line of sight to double-digit growth by the end of the decade," according to the company's quarterly announcements. Investors were more cautious on the day: CNBC reported that J&J shares fell 2% before the bell, after closing at $263.73 on Monday and rising 27% so far this year, outperforming both the broader market and most healthcare peers. The bottom line, by way of interpretation of these disclosed figures, is that the talc-related charge is a one-time hit concentrated in reported results — the $0.09 reported EPS versus $2.59 on an adjusted basis — while the raised sales and adjusted-EPS guidance reflects the company's own stated view of its underlying operations.
