Intuitive Surgical reported third-quarter 2026 revenue of $3.05 billion, up 22% from $2.50 billion a year earlier, driven by procedure growth, higher operating lease revenue and an expanded installed base, according to RTTNews. Shares fell after the company disclosed a charge tied to a proposed antitrust class-action settlement — put at $450 million by TradingView/GuruFocus — that dragged GAAP net income down even as underlying demand for its robotic surgery systems kept growing, according to TradingView/GuruFocus and Quiver Quantitative.
The $450 Million Settlement Charge
The proposed settlement still requires court approval, and Intuitive did not admit liability or wrongdoing in connection with it, according to TradingView/GuruFocus and Quiver Quantitative. The charge weighed on the bottom line: GAAP net income attributable to Intuitive fell to $523 million, or $1.49 per diluted share, from $704.4 million, or $1.95 per diluted share, a year earlier, per RTTNews, with Quiver Quantitative reporting the same per-share figures. On a non-GAAP basis, which excludes the settlement charge and other one-off items, adjusted net income rose to $1.05 billion, or $3.00 per diluted share, from $0.86 billion, or $2.40, a year earlier, RTTNews reported.
Procedure Volume and Systems Revenue

Reported procedure growth remained solid. Da Vinci procedure volume grew approximately 14% and Ion procedure volume rose approximately 31%, with combined worldwide procedures across both platforms up about 15%, according to RTTNews and TradingView/GuruFocus. Instruments and accessories revenue, which tracks closely with procedure volume, rose 18% to $1.80 billion from $1.52 billion, RTTNews reported, with surgeons running more than 10,000 procedures per day on average across the quarter, per 247wallst.com.
Systems placements also climbed: da Vinci system placements rose to 546 from 427 a year earlier, with 315 of those units being the newer da Vinci 5 platform, 247wallst.com reported. According to finsee.ai's earnings review, it was systems revenue, not procedure volume, that drove the acceleration in total revenue growth this quarter. Systems revenue rose 30% to $769 million, which finsee.ai said lifted total revenue growth to 22% from 19% in the prior quarter. The installed base reached 12,094 systems, up 12%, and 291 placements, or 53% of the total, were made under leases, finsee.ai said.
Finsee.ai also flagged a more cautious read beneath the headline numbers: in the US, 143 of 335 placements replaced an existing system rather than adding a new one, and by finsee.ai's calculation, each installed robot handled only about 1.5% more procedures than a year ago. That is an interpretive calculation from a single outlet rather than a company-reported figure, and it suggests utilization growth per machine may be slowing even as the fleet expands.
Margins and Capital Allocation
Non-GAAP gross margin expanded to 69.9% from 66.4% a year earlier, 247wallst.com reported. Finsee.ai noted that figure includes $28 million of refunded tariffs, and calculated that gross margin would have been 68.9% without that benefit, still an improvement year over year. Operating margin excluding one-off items reached 41.6% of revenue, up 2.7 percentage points, according to finsee.ai, which attributed the gain largely to sales and administrative costs falling to 17.7% of revenue from 19.0%, even as those overhead costs grew 13% against 22% revenue growth. Research and development spending rose 28% to $321 million, about half a percentage point more of revenue than a year earlier, per the same source.
Finsee.ai also calculated that diluted shares outstanding fell to 351.6 million, 2.8% fewer than a year ago, due to buybacks, which explains why profit per share excluding one-off items rose 25% while net income on the same basis rose 22%, a gap attributable to the lower share count rather than operating performance alone. Intuitive repurchased 7.8 million shares for $2.94 billion in the quarter, more than in the previous three quarters combined, finsee.ai reported, and ended September with $6.80 billion in cash, cash equivalents and investments, according to RTTNews.
247wallst.com reported that the company's outlook assumes current tariffs hold through year-end, with manufacturing exposure in Mexico and Germany flagged as a risk.
Share Price Reaction
Reports of the stock's decline varied by outlet and trading session. CNBC said shares slid 7% in after-hours trading as the results failed to impress investors. RTTNews said the stock closed the regular session at $438, up 1.11%, before falling 7.50% to $405.16 in overnight trading. A post from Investing.com on X said the stock plunged more than 13% even after adjusted Q3 earnings per share of $3.00 beat a $2.65 estimate and revenue topped forecasts. Quiver Quantitative separately reported the stock down 6.2% during a subsequent session.
Quiver Quantitative attributed the move primarily to the earnings release's legal charge, writing that investors appeared to focus on the hit to GAAP profit and the new litigation overhang even though underlying demand trends looked strong. TradingView/GuruFocus framed the reaction in expectations terms, writing that the $450 million settlement introduced a large one-time cost while investors are also watching whether procedure growth can maintain its current pace as competition in robotic surgery increases. Both framings are attributed interpretations from those outlets rather than company commentary.
Analyst Targets and What Wasn't Updated
Price targets catalogued by Quiver Quantitative predate this earnings release and should be read with that caveat: BMO Capital's Vik Chopra set a target of $518 on July 9, 2026; Evercore ISI's Vijay Kumar set $430 on July 6, 2026; and B of A Securities' Travis Steed set $520 on May 18, 2026, for a median of $518 across those three targets. None of these reflect analyst reaction to the settlement charge or the quarter's results.
Finsee.ai also noted several metrics investors track that were not updated in this release, including outpatient surgery center placements (27 in the prior quarter), cardiac procedure growth (39% in the prior quarter), Japanese placements (25 in the prior quarter), and any revenue figure for My Intuitive+ subscriptions. Their absence leaves gaps in assessing whether recent growth drivers, such as the cardiac specialty push and Japan's reimbursement expansion, are continuing at the same pace.
Bottom Line
Intuitive Surgical's core business, measured by revenue, procedure volume, system placements and margins, continued to grow at a healthy clip in the third quarter. But a $450 million charge tied to a proposed antitrust settlement, still pending court approval, cut sharply into GAAP profit and appears to have driven the stock's decline across multiple reported measures, even as adjusted earnings beat expectations. Whether procedure growth can sustain its current pace amid rising competition, a question raised by at least one outlet covering the results, remains open pending further disclosure in coming quarters.
